How to Build a Form

You can use our built-in Form Builder to create beautiful, functional forms or surveys using a simple drag-and-drop interface — no coding needed.

Click Add New Form or Edit to update an existing one.

The Form Designer

The screen is split into three main sections:

  • Left panel: Contains question types like text fields, dropdowns, checkboxes, and more.

  • Middle panel: This is your actual form — drag items from the left panel and drop them here.

  • Right panel: Displays settings for the selected question, panel, or page. This is called the Property Grid, where you can customize elements.

Add a Survey Title and Logo

At the top of your form, you can add a survey title and an optional description to give users context about what the form is for. You can also brand your form by adding your company logo. To do this, click the Picture icon at the top-right of the form — this will open your computer’s file browser so you can upload an image.

Elements of Your Form

A page is a container used to hold key parts of your survey, such as questions and panels. Every survey must have at least one visible page, and pages cannot be placed inside one another.


Within a page, you can use panels to group related questions together. Panels help you organise your form and manage multiple questions as a single unit. Unlike pages, panels can be nested — meaning you can place one panel inside another. This is useful for creating structured, complex forms and applying logic across question groups.

 

Question Types

Choose the kind of response you need when adding a question. Here are some examples:

  • Single-line Input: For short text answers.

  • Long Text: For longer responses or comments.

  • Checkboxes/Radio Buttons: Let users select one or more options.

  • Dropdown: A menu where users pick a single option from a list. Ideal for long lists with one answer.

  • Rating: Allows users to score something (e.g. service or satisfaction) using numbers, stars, or emojis.

  • Ranking: Lets users reorder a list based on preference or importance — great for identifying what matters most.

 

Putting It All Together

To add a question or element:

  • Drag it from the left-hand panel onto the form in the middle.
  • Or click Add Question in the middle panel.

Then click the question to customize it. You can update the question title by typing in the right-hand panel or by clicking directly on the question in the form.

You can:

  • Rearrange questions by dragging them up or down.
  • Set questions as required (marked with an asterisk *) by clicking the Required toggle on the question or in the right-hand panel.

Preview Your Form

Click the Preview tab to see exactly what your form will look like to respondents. This is a great way to check that everything looks and behaves as expected.

Save and Publish

Once you're happy with your form, click OK at the bottom of the right-hand panel to save it.

Your form can now be shared or embedded in BoardCloud meetings and agendas.

Respondents can use Next, Previous, and Complete buttons to move through pages and finish the survey.
