Form Register
The Form Register (located on the right-hand panel) displays all forms submitted by board members across meetings, committees, or standalone forms.
From here, you can:
- View a preview of a form
- Complete a form (if required)
- Open the associated meeting (if applicable)
- Open the associated committee (if applicable)
- View results in a table format
- View results in charts
- Email recipients
- Delete forms you no longer need
To manage a form, click the three dots next to the form name.
Icons on the My Forms Page
👤 Grey person icon – Form not yet completed
👤 Green person icon – Form completed
👤 Purple person icon – Who the creator of the form is
📅 Calendar icon – Shows the most recent date the form was created or edited
✔️ Green check next to form name – All assigned participants have completed the form
🚫 Circle with a line through it – Form is not anonymous
✔️ Green check (anonymity) – Form is anonymous
My Form Options 3 dots
- Preview Form: opens a preview of the form for you to view
- View Tabular Results: This will open a view of the results of the form as a table. There are options to download these results as a PDF, Excel spreadsheet or a CSV files
- View Chart Results: The Chart Results view allows you to visualize form responses in different formats, making it easier to interpret and present your data.
- Email recipients: Please select the recipients you wish to email.
These are the Form Recipients. Members you will be able to email the form to are limited to the members you selected initially. This sends an email to the recipients with a link to the form. - Delete form: Delete the form. Does not delete the initial form
